The following might be contributing to your problem.
You have all sorts of jquery errors on your signup page. The following is just an excerpt of the 44 odd lines of error logs I could see:

I have noticed something else – I can see the following error on your site when using the Chrome version of Firebug:
“Uncaught ReferenceError: ajaxurl is not defined”
Maybe this might be something related to your problem??

1. Can I use export to the excel based CSV file, and skip the google docs step, or is it still best to do the text based file and take it to google docs for some reason?
Yes definitely – doing it via excel is another equally good option. The google docs example was just a suggestion. As long you have a way of modifying and then saving the CSV file in the correct format it doesn’t matter which way you do it.
2. Will the export/import retain the product numbers that I have? I have a block of products that were removed, and so my product numbers have a gap.
Yes all of your product details (including the product ID) will be exactly as they were except for the parts which you modify in the step above.

Try the following:
1) Try deleting all YouTube cookies (tools > options > privacy > show cookies > youtube.com).
OR,
2) Try enabling “Accept Third Party Cookies” in FF if it isn’t already

No there isn’t a checkbox you can add to the shopping cart.
However the system already has this functionality built in because by default, mailchimp sends an opt-in email to your potential subscribers whereby they can elect to subscribe or not.
To make sure this occurs you will need to ensure that your “Disable Double Opt-In” flag is disabled (ie, NOT checked) in the Settings->Autoresponder Settings page

Strange thing is that the problem doesn’t appear to be occurring for the Opera browser but it definitely exists when using either FireFox or Chrome.
Are you using a caching plugin?
If you so have you tried this workaround?

This sounds like an unusual case. I’m not aware of any easy way to make eStore automatically create ‘n’ separate memberships where ‘n’ is the quantity of the item bought from eStore.
Perhaps you can use the “Product Specific Instructions” field to instruct your buyer to get in contact with you if they have bought multiple amounts of your membership product.
You can put this field in your automated email which gets sent after a purchase.
