It looks like you are using the following option:
Free Members Must Confirm Email Address
This option makes it so the form act in a specific way. Make sure you read the notes section of the following post to understand this option and how it interacts with the registration page.

Creating two separate estore products for this is the best way to go. Remember, you can have two add to cart buttons side by side (or below one another) on the landing page of this product.
With that said, you can use the digital variation option. This way the customer can choose if they want the digital or physical version.
For the shipping, you could use the shipping variation option:

What version of WP eStore plugin and Extra Shortcodes addon are you using?
There is an option in the manage customers menu to export a customer list. You can find this option at the bottom of the manage customers page.

I had a look at your site. The links are generating fine and when I email them to myself they are working and there is no extra code I use Gmail as my email client.
This would mean it is the email client the person is using to view their email is causing this issue.

Whatever commission you set for the affiliates, they will get it on the full sale. You cannot specify which month you want it to award the affiliate commission.
