The proper way to do this is to have terms and conditions on your registration page. So when they are registering, they are agreeing to your terms (once). Then any continued use of the site is covered by that terms. YOu don’t really want to allow them to even register if they don’t’ accept your terms.
There is an option to show terms and conditions in the registration form.
The textboxes can be made wider using CSS tweaks. If you show me the page containing the squeeze form, I will be able to inspect the HTML and offer CSS tweak for it.
It is likely that plugin executed the email sending code fine but the email got blocked somewhere.
The log file will say if the expiry email notices were executed by the plugin or not. Do you see anything like the folloiwng in the conjob log file?

Custom jobs are generally tricky. We get to it on a lower priority basis. If something we advertise needs attention, it gets higher priority.
When the user goes to the Taxcloud interface, isn’t there an option where the customer can select, Non-US and then they don’t get charged anything?
