New affiliates that have signed up cannot access their account. They receive an email confirming the signup but that is all that happens. I do not see them in my Manage Affiliates area. I am able to sign them myself through the WP-Affiliate add/edit within WP.

I just signed up for two test accounts and I was able to confirm the problem you are talking about.
At this stage I’m not sure what may be causing it but please be patient until Admin sees this thread and he will suggest the next course of action.

November 27, 2012 at 12:15 am #51314adminKeymasterNothing will happen to your settings and affiliates list and any other data that is saved in the plugin. So please go ahead and load a fresh copy of the plugin then check it.
November 28, 2012 at 6:34 pm #51315pregenMemberTried the refresh copy – no effect. Turned on PHP error logging and got the following error:
[27-Nov-2012 01:32:16] WordPress database error Field 'account_details' doesn't have a default value for query INSERT INTO wp_affiliates_tbl
Putting empty values for the columns account_details and sec_tier_commission_level are sufficient for us for now.
Is this an error in the column definition (not having a default value) or just the missing column in the INSERT?
Hope this helps. We are running satisfactorily for now. Thanks for looking into this.

Normally, you don’t have to specify the value of a column if it is not mentioned in the insert statement. However, a particular variation of Windows server throws a fit with it somethings (this is the effect you are seeing).
Downloading a new copy of the affiliate plugin and updating it should have taken care of it. Something definitely went wrong with your update because the code you shared is old code which tells me that the code on your server is not the new affiliate plugin code.
