I have tried time and again and for some reason the PayPal Profile shipping is not working. When customers order items, the shipping fees are not included.
Please take a look at the following posts that have good info on this topic:

eStore – Paypal Shipping profile no longer triggering after upgrade
If that doesn’t help then please post a link to your products page so I can check a few things. Are you using dollar based or weight based profile shipping?
