Can someone help me make this update? I am not a developer and I need to make sure my affiliates are getting recurring commissions. help!

Hey I have the convertkit extension and I just tested it and it did not work. Can you confirm if I need to put in the sequence name or the form name? I have tried both now and nothing went into convertkit
