I linked an image of my test csv file which shows all the column names and the data row of the one entry. I did ensure they were identical to your example template.[thepags.org/wp-content/uploads/2020/12/TipsRequest-1.jpg]

I am using UTF-8 for the csv but I have found the following from a support notice issued by my hoster, WordPress.com.

Some plugins require a particular database character set, such as UTF-8. Sites on WordPress.com use Latin1 as a character set. If a plugin requires a different character set, please reach out to the plugin developers for an alternative solution.

Perhaps that is the problem then?
