I have deactivated every other plugin and activated the standard theme but “lastname” still displayed…

Peter, I really can’t find the problem. I dont’ use any chaching plugins… Can you take a look inside my blog?

Thanks for your answer. I have lastname unchecked but it still shows lastname… What do I wrong or what do I have to change to fix this problem?

I don’t understand why I have 2 check boxes under Pages/Forms Settings…
