PayPal Profile Based Shipping is not working. When I have the PayPal Profile Based Shipping box checked in Settings, I have no place to select zip code when I’m sent to Paypal. If I don’t have PayPal Profile Based Shipping checked, the zip code box shows up, but shipping costs are $0.00. I only have 1 product setup because I want to make sure this works before I complete all my products:

Any suggestions?
It looks like you have that product set as a “Digital” item. Edit the product in question. Then look at the “Digital Content Details” section. Is the “Downloadable” checkbox checked? If it is, please uncheck it. So the plugin can treat this product as a physical item.
