Hi,
I would like to customize the offline/manual payment portion where the users will select the offline payment mode they will be using (for example, “Cheque” or “Bank Transfer”).
What is the best way of doing so?

Please read the following post for some general info on the manual checkout process:
WP eStore's Manual/Offline Checkout Methods (how manual checkout works)
The customer actually doesn’t need to select it since the payment will be done manually.
Go to the following settings option:
WP eStore Settings -> Payment Gateway Settings -> Manual Checkout -> Directions for the Customer
Now add instructions for both payment options via “Bank Transfer” and “Cheque” (Basically telling them how they can pay for the items). The customers will get that info upon manual checkout. They can then make the payment using the method that they prefer. When you receive the payment, you give them the items they ordered.
Thanks! In this case, there’s a form which the user will need to fill in upon manual checkout. Can I create an additional form field and how can I save the record and display it out accordingly?

There is a field called “Comments” in the manual checkout form. Users can add any info there. Can you use that field to collect some extra info from your users?
HI, I was thinking of adding radio groups so that admin end it will be easier for some other data collection.
