Manually Approve Member Registration for WP Affiliates

Two quick questions:

1. I’m looking for a way to manually approve new affiliates after the sign up. I’ve read in the FAQ that this is available but I’m not seeing it in the Settings anywhere.

2. I also need to add some custom fields to the sign up form and found how to do this, however, when I got to the WP affiliate plugin settings, there isn’t a link on the right for Custom Fields, I only have: “General Settings, Email Settings, Autoresponder Settings, and WP User Settings”. Please let me know how to get this setting/option.

Regarding custom fields – there is no option to add custom fields to the affiliate signup form.
