I want to create a membership website with 3 levels. The auto-renew period is 30 days. If someone, on one of the levels, decides on day 17 (for example) to upgrade or downgrade, how is that handled via eStore and Paypal?
-Do they have to pay again in full when starting the new level? If so, does this system automatically calculate a prorated refund then send that refund to the member for their previous level’s payment?
-Or will the system know how long into the 30 days this membership is and ask for payment for the remainder of the 30 days for the next level?

eStore can’t do pro rated cancellation. Please read the following two posts which should help with your question:
Ok thank you for the links. On the same topic, if a member switches to another level, do the estore/emember plug-ins cut-off the previous membership right away, while opening the new membership tier? Or does the previous membership tier continue on until the expiration date is set, having then, two memberships tiers running in parallel (if you know what I mean?) ?

If you are not using the “Multiple Membership” option then yes it will switch to the new membership immediately and the access to the old membership level’s content won’t be there.
