It is definitely missing the following image file:

    wp-affiliate-platform/affiliates/images/register.png

Do the following to make sure everything gets uploaded:

Ok, have reinstalled plugin. Same issue. Checked images folder for register.png and discovered that it is present. So error must be in the php get action.

No, the error is not in the plugin for sure. This same plugin is used on thousands of sites (including our site).

Something on your site is preventing accessing that image file. Ask your hosting provider why the following image URL gives a 404 error (enter this URL in your browser to see what I mean):

I disabled the All in One WP Security plugin and the problem went away. I then enabled the basic security settings and it came back. So I’m betting it has something to do with this plugins firewall protection settings. I’ll tinker with the settings to try to narrow down exactly which setting causes this issue.

It was the “Advanced Character String Filter” under the Additional Firewall Rules. I disabled that one and everything’s fine. Is not this plugin yours?
