Is there way that I can add it by myself? Bcs seems like the plugin can pull the info for names & email. I guess it can be done by adding the Id of the field right?

Thank you for your respond. May I know which file in the plugin that I should edit to customize it to display the extra field? I just need to add that one extra field.
