I have the box ‘Manually Approve Member Registration’ checked and initially the notification emails have been triggered correctly. However I believe that the latest upgrade to WP 3.3.2 has broken the notifications. What is happening is that the ‘Your account has been activated’ notification is going out immediately the member completes their registration. The email is incorrect because activation still requires manual intervention.
Is there a fix for this, is it a known issue?
The email logs below show the problem.

The latest WP upgrade has nothing to do with (we already tested everything with this WordPress update).
The “Registration Complete” email is sent to the member after they complete the registration. Their account won’t be active until you manually approve it though. The only way to send the “account activated” email is by editing the member’s profile from admin dashboard and use the “Send a Notification to the User” option to send the notification email. No automatic notification is sent out just for activating an account. So you must have edited that member’s account from admin dashboard and used that option to trigger this email.
Sorry for the late reply, your explanation helped me get the results that I needed.
Thanks
