WP eMember: What leads to error message "failed attempt to register"?

A client’s customer was trying to register and got the error message “failed attempt to register.” I have not seen this message before.

I registered her easily with no error messages. This leads me to believe there might have been an error on her part, but I don’t know what it may be.

Under what circumstances does the “failed attempt to register” message appear?

The membership plugin doesn’t have that error message anywhere in the plugin. This is not something coming from the eMember plugin.
