Squeeze form – add tick box to agree to be added to mail list

I need to add a tick box to the form so anyone downloading using the squeeze box can tick it to agree to being added to a mailing list. (this is to comply with the Canadian anti-spam law CASL)

I just want to add the tick box and short text, not a link to a T&C web page which is currently what happens now.

Is there a solution?
