Simple Shopping Cart – Customize the email to seller

I would like to gather some additional information on the checkout page and have it included in the email to the buyer and seller.

How can I do that?

For example: You have a couple of items in the cart and below the items would have a couple boxes to fill out. Like contact phone, name, etc… I need about 4-5 fields. I don’t need the fields to go to Paypal. Just be sent in the email.
