Sagepay payment gateway not creating a transaction

We have the estore and gateway plugins and added sagepay.

We were not getting the order confirmation emails but are getting a payment confirmation from sagepay.

We added Paypal as a gateway and that works.

Then we noticed that the sagepay orders are also not registered as a transaction or customer (the Paypal are).

ipn_handle_debug only shows the Paypal transactions.

The only obvious omission is Cart ID?
