Receiving 404 Error Code after affiliate attempts to sign up

Recently setup WP affiliate platform and I’m having issues with the sign up form – at first I had the option checked to add affiliates to my GetResponse campaign which was producing:

Fatal error: Uncaught exception ‘Exception’ with message ‘Request have return error: Array’

Unchecked the option and then now I’m receiving a 404 error.

I moved my affiliates folder to [www.mydomain.com/affiliates] and it appears the script is trying to call the thank you page from it’s original location in the plug in folder.

Instead of moving the affiliate folder, do “option 2” mentioned in the documentation below:
