Quantity Field align left, item number does not show

on checkout in the shopping cart the item quantity does not show, the field is to small and aligned to the left.

Please advise how to change it.

Also after successful checkout with offline payment it confirms the purchase but also brings up the message: Your shopping cart is empty. Please advise.
