It’s for a gift type order where the user needs to be able to provide an email or address for the gift to be sent. http://www.sizzlpix.com/main/gift-certificates/

You can use the “Collect Customer Input” field that is under the “Additional Product Details” section of the product to collect customer input for a product.
You can also use this “Collect Special Instruction” feature of PayPal to achieve this:
