I would like to create a new registration on my site that will allow users to choose what type of membership they want from a dropdown or checkbox.
All the memberships on my site are free, they simple allow access to additional content based on their membership.
This is the page where I will be adding the new form allowing new users to choose their own membership level:

I have read both of these articles already but they do not solve my issue.
I already have 5 separate forms for users to register and the different membership types.
I want to create one form that will allow users to choose which membership type they want.
Are you saying this is not possible?
We can’t have a drop down for membership level in the form (there are a few reasons for it).
Do the following to get a similar result:
1) Create a new page.
2) Add 5 links to the 5 different pages you created for your levels. Add some explanation for each level also.
3) Now, send users to this newly created page. So they can select the level they want and then go from there.
You can even make your own dropdown box on this new page. Depending on the selection, you just redirect them to the appropriate registration page.
You can do the same thing on your join-us page also. So the user will select the level they want to signup from your join-us page which will send them to the appropriate page.
