Lightbox Ultimate – Server error when trying to enter shortcode

I’m trying add a self-hosted video to a WordPress page, linked from the WP media directory. When I attempt to enter shortcode into the editor I get a popup error message: “Forbidden: You don’t have permission to access /wp-content/plugins/wp-lightbox-ultimate/lib/wp_lightbox_ultimate_shortcode_insert_window.php on this server.”

In troubleshooting I removed the site login (htaccess).
