lightbox ultimate – no options on tinyMCE for shortcode inserter

I have successfully installed this plugin. When I go to use the plugin with the tinyMCE visual editor, clicking on the button leads to an empty pop up appearing and nothing loading in it.

I cannot find any getting started or how to guides to try and problem solve this.

I am using a thrive theme. The plugins I have installed are pilotpress (a membership plugin from Ontraport), thrive clever widgets, thrive visual editor, thrive leads, wordpress importer and reveal IDs.

Any work throughs for this?
