License model for eMember Form Builder Add-on?

Is the license for the eMember Form Builder add-on the same as for eMember, eStore, etc?

The offering page:

doesn’t seem to say precisely, although I’m guessing that it follows your typical model.

The Form Builder Addon, follows the base eMember license. Other Addons, such as the Gateway Bundle, are “Single Install,” because of 3rd party licensing requirements, and are labeled as such, right before the “Grab It Now” button.
