How to add more custom input fields

I would very much like to have the ability to add 2 or more “Collect Customer Input” fields. OR to use the variations (1,2 or 3) to allow input. How can I do this?

For example:

List Dietary Needs: (user can input their allergy, or restrictions)

List Roommate Preferences: Hannah, Suzie or Mary

The custom input field is passed back/forth between eStore & PayPal. As such, there is a limit on how much information can be transferred. The following post, offers a further explanation; along with some “options” you can consider:
