How to add field to download squeeze form?

Hello!

We would like to have a small set of free downloads via squeeze form where we ask for name, email AND location (open text).

Is there a shortcode that allows a custom field to be added to the squeeze form? And if not, is there a way to create this on our end?

We don’t need it all the time – though we might not mind it if there’s no other way to get it into our shortcode.

We use a child theme – and could make plugin edits that we maintain separately in order to restore customizations after upgrades – which we already do for our estore plugin english language files.
