Adding that CSS code to my custom CSS hid the name field from the squeeze form until I updated to the most recent Genesis framework. When I updated, the custom CSS stopped working to hide the field.

Any idea why the name field showed up again?
Did you enter that custom CSS in any of your theme’s file? If you did it probably erased it when you did the upgrade. Did you try entering that custom CSS again?
It is better to use a plugin like the following to enter custom CSS so it doesn’t get overwritten:
