eStore checkout form needs two checkout boxes

Greetings Beautiful WTW Family. Question please:

1.) I am a coach.

2.) I am building a new website.

3.) The “Tips & Tricks” paid for “Paypal Cart” Plugin works awesome. Thank you !!!!

4.) Because I am a Coach, my clients have to sign or agree to the “Coaching Agreements” before I am able to sign them on.

5.) The “Paypal Cart” only allows one checkbox for the “Terms&Condition” . I need two check boxes before they purchase.

6.) Does anyone have any ideas how to create an extra checkout box for “Coaching Agreements” to be cheked, before the client checks out?
