Empty or Blank Details Page when using Manual Checkout

Hi,

We have our shopping cart set up at http://www.eBookChannel.info and we’re using it to offer a variety of free ebooks and so we are using the manual checkout process to allow us to gather names of those people downloading the eBooks.

When I test it though, all works fine (I’m using the How to Read Her Mind ebook), I add it to the cart and then go to checkout, when I do though the details page is blank?

Any thoughts? We’re using the Headway theme and it’s still blank whether it’s in Chrome or Firefox, I haven’t tested in IE or Safari though. Maybe some code adjustment? If so could you provide that for me, or give me a recommendation.

The code for the form doesn’t seem to be there. Have you done any modification to the plugin? Get a fresh build from here and give it a try:

Okay i’ll try that, but I haven’t done any modification to it, but it just might need an update. I’ll let you know if I have any other issues.
