I would like the labels to show above or next to form fields, instead of just the placeholder text. Is this possible?

Just recently we made an update to the plugin to move away from that structure. The new structure is better for responsive design (it works better in all devices). Maybe we can introduce the old form style as an alternative via a different shortcode.
