Customer Input Field not working

Hello!

I’ve enabled the Customer Input Field and added a label, but it does not show.

Check http://www.saugusfootball.com/gridiron-club/spirit-wear

The product is the 2-for-1 tshirt item.

When you edit the product in question is the custom input checkbox still checked?
