Collect Customer Input email tag

I have the Collect Customer Input configured to capture the customer’s phone number. How can I use an email tag in the notification to seller email to include this field? Is there a better way for me to collect the customer’s phone number and have it emailed to me?

By the way, I an using WP eStore and WP eMember.

The value from the collect customer input field will be added to the product name. So all you have to do is look at the product name in the sales notification and you will be able to find this value.
