Code to create a customized login form

Hi, I did recreate my website (at the moment you can verderlo here), making achieving a bar placed above the header to login. I need to know what is the code to insert so that users can use the scroll bar above the header and instead use the shortcode in other pages.

When a user clicks on Login, which code must be entered to let him log in? I have to pass this information to the web master who is carrying out the work
