Clicking link to play mp4 does nothing regardless of type of shortcode used

I just purchased Lightbox Ultimate and installed according to instructions on your site using the WordPress plugin uploader. I am running WordPress 3.5.1 and Lightbox Ultimate Version 1.9.3.

I entered shortcodes by using the examples in “WP Lightbox Ultimate Shortcode List by Library Type.pdf” on a test page. You can view it here: [http://gsship.org/test-page/]

Only once did the lightbox appear, (I believe when I tried the flowplayer shortcode) and even then, no video displayed. I tried three different shortcodes, see below.

All three are on the test-page now.

I searched the forum and tried uninstalling and reinstalling according to the directions with no change in results. I also searched the source code for multiple jquery.js files being loaded and did not find any.

The Lightbox_prettyImage shortcode works with an image from my site. So, I am not sure where to look.

I am using W3 Total Cache for cahce plugin and using a CDN for content delivery.
