It can also have a label of “Note to seller” and there is a link called “add” that you can click to add nots/instructions. Do you see anything like that?
If you add an item from here then click the checkout button (don’t do the full checkout) you will see how it shows up:

Also, you maybe not have this enabled in your PayPal merchant account profile. So you may want to check your profile settings area in your PayPal account. Or Ask PayPal if the merchant’s country has anything to do with (PayPal doesn’t give some features to certain countries).
