Video Lightbox – 640×480 mp4 video not playing?

I have a video that will not stream and I do not understand why. Perhaps you can help.

Here is my annotated test page. It should be completely documented.

Why does not the bottom one play? They are both about the same size, same format and are playing out of the same depo. Just the aspect ratios differ.
